Heritage Case Management (HCM) provides comprehensive case management to seniors and adult disabled individuals through an assessment of the clients needs resulting in the creation of service plans which are monitored and implemented. Case management is an ongoing collaborative approach to coordinating and integrating healthcare services.

HCM and its multidisciplinary team of qualified professionals are committed to excellence and the provision of quality care while empowering the consumer to make informed choices about their health care and the consumption of medical services.

HCM's team posses detailed knowledge of the multi-systemic continuum of care including acute, rehabilitative, long term, community, spiritual and alternative resources. HCM bridges gaps by helping clients and families to understand the complexities of the health care delivery system by assuring the provision of needed information and making appropriate referrals to healthcare providers on an ongoing basis.

HCM's professional staff works with the client and their family to identify and facilitate the delivery of the services from the long term care continuum, necessary to meet the client’s needs at home or during transition in/out of healthcare facilities.

HCM is fully licensed by the State of New Hampshire to provide medical case management services throughout the state and operates in accordance with all applicable federal, state and local laws, regulations and ordinances.
